Translating the Latest KDIGO Guidelines Into Practice: IV Iron Therapy in Patients With Non–Dialysis-Dependent CKD

  • Overview

    Iron deficiency and iron-deficiency anemia are related but distinct clinical entities, with iron deficiency often preceding or occurring independently of anemia. In chronic kidney disease (CKD), inflammation-driven iron sequestration and impaired iron availability frequently develop before hemoglobin declines, making hemoglobin-only screening insufficient. Recent updates to the KDIGO 2026 Clinical Practice Guideline for the Management of Anemia in CKD highlight the expanded role of IV iron replacement therapy in patients with non–dialysis-dependent CKD, along with the need for proactive iron assessment, earlier identification of iron deficiency phenotypes, and individualized selection of iron replacement strategies to improve long-term patient outcomes.
